Inequality (factual) simply uneven distribution of health as result of genetic or other factors or lack of resources. But inequity is unfair, unjust and avoidable differences because policy, governance, corruption or cultural exclusion. 

According to data from 1960 to 2000, black-white gap has caused 83,000 avoidable deaths African Americans. This is related to social determinants, neighborhood ecology, provider bias, structural inequities, and institutionalized discrimination. (according to David Satcher et al. Health Affairs Journal)
